The book’s preface notes that: “The authors’ names on the front of the book have been placed in the order they appear in the chapters. The order of names does not represent a hierarchy of importance or reflect inequalities in contributions to the book. All are essentially equal as authors, though publishing and citation conventions require that the names be used in one order on a consistent basis.”
